SURPLUS LINES BROKER TAX FILING FORMS & INSTRUCTIONS

PURSUANT TO ARKANSAS CODE §23-65-301, ET SEQ., AND RULE 24 ON SURPLUS LINE INSURANCE

Use this list to choose a broker for your filings if you are not licensed in Arkansas as a surplus lines broker.

Remit the 4% broker’s tax by the required due dates to avoid incurring monetary penalties.  To facilitate timely report and tax filings, please adhere to the following chart for a calendar year:

 Rule 24   Surplus Lines Insurance

FORMS

Quarterly:

     1.  Instructions Surplus Lines monthly filing information

     2.  Affidavit of Surplus Line Broker

  • The person signing this form MUST be licensed in Arkansas as a surplus lines broker.

     3.  SL-2 & SL-2a Quarterly Statement of Surplus Lines  This form is designed to be completed in Excel, uploaded, and submitted in OPTins with tax payment.

NOTE: If a surplus line broker has a “zero” quarterly filing, the broker is not required to send a form to the Department.

Annual:

     1.  Instructions  Surplus Lines annual filing information

     2.  SL-4    Affidavit 

  • The person signing this form MUST be licensed in Arkansas as a surplus lines broker.

     3.  AR_SL-4  This form is designed to be completed in Excel, uploaded, and submitted in OPTins even if it is a zero "0" filing. (due by March 1st each year.)

 Annual forms must be completed in Excel, uploaded, and submitted in OPTins even if no premiums were written.

 Forms to be kept on file in the Surplus Line broker’s office:

      1.   SL-3    Disclosure to Surplus Line Insured 

  • This form is not required to be filed with the Insurance Department, but is required to be available if requested by the Commissioner.
